Below is a checklist adapted from Strengths and Difficulties Questionnaire (Copyright, Robert Goodman). For each item, please mark the box for Yes, or No. Please give your answers on the basis of the child's behavior over the last six months or this school year. If you have 2 or more answers in "Yes" in any of the symptoms scale, please consult the Counsellors.
1. Emotional Symptoms Scale
Often complains of headaches, stomach-aches or sickness
Many worries, often seems worried
Often unhappy, down-hearted or tearful
Nervous or clingy in new situations, easily loses confidence
Many fears, easily scared
2. Conduct Problems Scale A
Often has temper tantrums or hot tempers
Not obedient, usually does not do what adults request
Often fights with other children or bullies them
Often lies or cheats
Steals from home, school or elsewhere
3. Hyperactivity Scale B
Restless, overactive, cannot stay still for long
Constantly fidgeting or squirming
Easily distracted, concentration wanders
Does not thinks things out before acting
Does not see tasks through to the end, good attention span
4. Peer Problems Scale A
Rather solitary, tends to play alone
Does not has at least one good friend
Picked on or bullied by other children
Gets on better with adults than with other children
5. Prosocial B
Not considerate of other people's feelings
Does not share readily with other children (treats, toys, pencils etc.)
Not helpful if someone is hurt, upset or feeling ill
Unkind to younger children
Does not volunteer to help others (parents, teachers, other children)
